Public Information Request

The Creedmoor Maha Water Supply Corporation provides several ways to submit a Public Information Request. The request form can be viewed, prined, or downloaded below.

• Hand delivery or mail - You may request a form from our office and submit it in person or or by mail.
• Writen Request - You may submit a written request to any authorized staff member by email or letter. The completed application form must be included. 

Please note that for a public information request to be valid, it must be submitted in writing.
